Please do not continue with the registration process if it advises you that you are out of our catchment area.
Please inform us about a change of name, contact number or address as soon as possible.
If you move out of the practice area, you will be required to register with a more local practice.
How to Register as a Patient
If you live in our practice catchment area (see above) you can join our list. If you are registering your new baby please check the next section.
Registering with us Online
We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one (see below).
Registering via Paper Form
To register, please complete the Register with a GP Surgery (PRF) form and return it to the practice along with a New Patient Registration Pack. This application form provides us with vital information about our patients.

You do not need proof of ID to register with a GP, but it might help of you have one or more of the following:
- passport
- birth certificate
- HC2 certificate
- rough sleepers’ identity badge
- hostel or accommodation registration or mail forwarding letter
If you are homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.
How to Register your New Baby
You need to register your new baby at the practice as soon as possible. Therefore please download and complete a General Medical Services (GMS1) Registration Form (also available from reception) and send it to your surgery. Please ensure you provide your baby’s NHS Number which will be detailed in their Red Book. Once we have received the form we will register your baby as a patient at this Practice.
The Practice will send you an appointment for a mother and baby post-natal check. This is a 50 minute appointment, 30 minutes for the baby check and 20 minutes for the post-natal. Your baby’s course of routine immunisations will start at 8 weeks and will be given at this appointment. Please remember to bring the red book. If you cannot make this appointment you must notify reception as soon as possible. Please be advised that if you do need to cancel, the next available appointment may be anything up to 4 weeks after that date.
Temporary Residents
The Doctors are happy to see patients who are temporarily resident in the Practice area should urgent medical care is required. In this instance persons wishing to be treated should state that they are a temporary resident requiring immediate and necessary treatment when reporting to reception.
For non urgent problems, we strive to accommodate patients who are temporary resident in the Practice area but we may not always be able to do so on the same day if there are no routine appointments left. In this case, we would either offer a routine appointment in near future or provide a list of local practices whom you may wish to contact.
To register you will need to complete our Temporary Resident Registration form (GMS 3) which is also available from reception.
Accountable Named GP for All Patients
All patients registered at Wolstanton Medical Centre have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.
If you are unsure of who is your named Doctor please ask at reception.
New patients are allocated a Doctor on Registration.
